Apgar Score Explained

The Apgar score is a quick 0-to-10 check of a newborn's condition at 1 and 5 minutes after birth. Here is what its five components measure, and what a low score does and does not mean for your child.

The Apgar score is a quick, standardized check of a newborn’s condition in the first minutes of life, scored from 0 to 10 across five signs and recorded at 1 and 5 minutes after birth. It was designed by anesthesiologist Dr. Virginia Apgar in 1952 as a fast, repeatable way to tell which babies need help breathing and circulating, rather than as a verdict on a child’s future. That distinction is the single most important thing for a worried parent to understand, and this page keeps returning to it.

What the Apgar score is

Every baby born in a U.S. hospital is assessed with the Apgar score. A nurse, midwife, or physician looks at five signs, gives each one a score of 0, 1, or 2, and adds them up for a total between 0 and 10. The assessment is repeated, so a single baby gets more than one number: routinely at 1 minute and 5 minutes of life, and again every 5 minutes up to 20 minutes if the early scores are low. The name is both the inventor’s surname and a teaching mnemonic: Appearance, Pulse, Grimace, Activity, Respiration.

The American Academy of Pediatrics and the American College of Obstetricians and Gynecologists are explicit about what the score is for: it is a description of the baby’s condition at a moment in time and a way to gauge the response to resuscitation. It was never intended to predict long-term neurological outcome, and it should not be used that way. Keep that sentence in mind as the numbers below get more specific.

The five components, scored 0 to 2

Each sign is scored independently. Here is exactly what earns a 0, a 1, or a 2:

Two of these (heart rate and respiratory effort) are the ones clinicians act on most urgently, because they drive the decisions of neonatal resuscitation. A perfectly healthy newborn often scores 9 rather than 10, because a baby’s hands and feet commonly stay bluish for a while after birth (acrocyanosis) and lose a point on Appearance. A 9 is not a warning sign.

What the 1-, 5-, and 10-minute scores mean

The scores are taken at different times because they answer different questions.

  • 1-minute score: a snapshot of how the baby tolerated the birth itself and whether help is needed right now. A low 1-minute score is common and, on its own, is not alarming; many babies who score low at 1 minute are completely fine moments later.
  • 5-minute score: the more meaningful number, because it reflects how the baby is responding to any help given. This is the figure clinicians and later reviewers pay closest attention to.
  • 10-, 15-, and 20-minute scores: recorded only when earlier scores are below 7, to track the response to ongoing resuscitation. A score that stays low over these later checks is more concerning than a single low number early on, because it means the baby is not recovering as expected.

A widely used shorthand groups totals as 7–10 (reassuring), 4–6 (moderately depressed, may need help), and 0–3 (severely depressed, needs immediate resuscitation). Treat these as rough bands rather than sharp lines. What matters clinically is the trend across the timed checks, more than any single digit.

What a low Apgar does and does not predict

This is where precision matters most. A low Apgar score is a signal that a newborn needed help: nothing more, and nothing less.

What it cannot do: it cannot diagnose brain injury, measure intelligence, or forecast whether a child will have a disability. The overwhelming majority of children with low 1-minute scores grow up neurologically normal. Even among children who go on to develop cerebral palsy, most had normal Apgar scores at birth, and most children with low scores never develop cerebral palsy. The AAP and ACOG have stated plainly that the Apgar score alone cannot establish that an injury was caused by a lack of oxygen around birth.

What it can flag: a 5-minute score of 0–3 that persists at 10 minutes and beyond is associated with a higher statistical risk of neurological problems, but a raised risk across a population is not a prediction for any one child. A persistently very low score is a reason to look harder rather than a diagnosis in itself. Apgar and HIE: why the number alone is not a diagnosis

Parents often arrive at the Apgar score while trying to understand a possible diagnosis of hypoxic-ischemic encephalopathy (HIE), the brain injury caused by oxygen deprivation around birth. A low Apgar is one piece of that picture, but the diagnosis is never made on the score alone. Clinicians and, later, medical reviewers look at a constellation of objective findings together:

  • Umbilical cord blood gas results, specifically a low arterial pH (often cited around 7.0 or below) and a base deficit indicating significant acidosis
  • The need for and duration of resuscitation, including whether the baby required a breathing tube
  • Signs of encephalopathy on neurological exam and the Sarnat staging that grades its severity
  • Seizures, abnormal tone, and feeding difficulty in the newborn period
  • MRI findings and, sometimes, evidence of injury to other organs

This is why a cord blood gas drawn at birth is so valuable, and why parents are encouraged to make sure it was obtained. The Apgar score sits alongside these measurements; it does not replace any of them. A low Apgar score is not, by itself, evidence that anyone did anything wrong. Babies are born depressed for many reasons, plenty of them nobody could have prevented. Not every difficult birth is malpractice, and a low number is not a lawsuit.

Where the score becomes legally relevant is as part of a larger timeline. Reviewers ask what came before the low score (was the fetal heart-rate tracing showing distress that went unaddressed for an hour? was a needed cesarean delayed?) and what came after: was the resuscitation prompt and competent, were cord gases drawn, was HIE recognized in time for cooling to help? A low Apgar paired with a category-III fetal tracing that was ignored, a severely acidotic cord gas, and a delayed delivery tells a very different story than a low Apgar in an otherwise unremarkable birth. The score is a thread, not the whole cloth. Sorting out which one you are looking at is exactly what a records review does, and it often brings the reassurance that nothing was missed.

Extended Apgar during resuscitation

When a newborn is being actively resuscitated, the AAP recommends recording an extended Apgar, continuing the timed scores past 5 minutes (at 10, 15, and 20 minutes) alongside a note of what interventions were underway at each check: oxygen, positive-pressure ventilation, chest compressions, medications. This matters because a baby on full support may score low for reasons that reflect the treatment as much as the underlying condition, and documenting the interventions keeps the number in context. An extended Apgar that stays very low despite vigorous, correct resuscitation is a more serious sign than an early low score that quickly improves. It is also, for parents seeking answers later, one of the more informative parts of the delivery record.

Frequently asked questions

Is a low Apgar score dangerous?

A low score means the baby needed help at birth, and it prompts the care team to act. On its own it is not dangerous and is often temporary: many babies with low 1-minute scores are completely healthy minutes later. A score that stays very low across the 5-, 10-, and later checks is more concerning and warrants close evaluation.

Does a low Apgar score mean my child will have brain damage or cerebral palsy?

No. The Apgar score cannot predict brain injury, cerebral palsy, or intelligence. Most children with low scores develop normally, and most children who do have cerebral palsy had normal Apgar scores at birth. A diagnosis requires other findings (cord blood gases, imaging, and neurological exam), never the score alone.

What is a normal Apgar score?

A score of 7 to 10 is considered reassuring. Many healthy newborns score 9 rather than 10 because their hands and feet stay bluish for a while after birth, which costs a point on skin color. That is normal.

Why did my baby have two different Apgar scores?

Apgar is measured at more than one time: routinely at 1 and 5 minutes, and again at 10, 15, and 20 minutes if the early scores are low. The scores usually differ because they capture how the baby is changing and responding to any help. The 5-minute score is generally the more meaningful one.

Should I be worried if the cord blood gas was not drawn?

A cord blood gas is one of the most objective records of how a baby tolerated birth, and it is valuable when questions arise later. Its absence is not proof of anything, but if you have concerns about the delivery, it is one of the specific items worth asking about when you request the complete records.
